All Request for Purchase Order Forms MUST be typed!  Handwritten orders will NOT be accepted.

Tips to remember while completing a Purchase Order Request:

• If you put an amount in the "Unit Price" column, you must also put a quantity in the "Quantity/Unit" column for the P.O. to calculate an amount in the "Total" column.
• If you list "as attached" in the Description column, or you have any attachments you would like to include with the P.O., you must submit three (3) copies of the attachment with the P.O.
